Explanation of Terms Used
'We' means the business of Orriss & Low Optometrists.
'You' means the person named on the contract. If that person is under 18 their parent/s or guardian are responsible to ensure they keep their side of the contract. 'The Practice' means the premises of Orriss & Low Optometrists, at 2/4 Corner House, Woodborough Road, Winscombe BS25 1AQ.

We Undertake to:

• Provide a Sight Test every year.

• Provide emergency advice/appointments as required, where clinic space allows (excluding any related to contact lens wear).

• Give one calendar month's notice of changes or termination of this agreement in writing.

• Give 20% discount on new prescription spectacles/sunglasses and prescription lenses.

• Give 15% discount on other products, including non prescription sunglasses.

• Give a minimum of 1 month's notice of any price increase.

In addition GOLD Members receive:

• One Comprehensive Eye Examination every year.

•Extensive Optomap & OCT Scan examinations as required.

• Carry out additional diagnostic examinations as required.

• An EXTRA 5% discount on new spectacle frames.

You Undertake to:

• Attend for eye examinations as determined by your Optometrist.

• To attend for appointments made, and pay for missed appointments or those cancelled with less than 24 hours notice.

• Comply with all advice given in writing, and verbally.

• Pay regular amounts by Direct Debit for a minimum of 18 months, from the first payment date. Missed payments will carry a fee. If a monthly fee remains unpaid 1 month after the due date we may terminate this contract and services immediately by giving you written notice.

• Give 2 month's notice, in writing, if you wish to cancel this agreement after the minimum term of 18 months.

• To provide an up to date email address for administration purposes.

(Including Contact Lenses)

'You' means the person named on the contract. If that person is under 18 their parent/s or guardian are responsible to ensure they keep their side of the contract. 'The Practice' means the premises of Orriss & Low Optometrists, at 2/4 Corner House, Woodborough Road, Winscombe BS25 1AQ.

We Undertake to:

• Provide routine contact lens checks, at intervals as advised by your Optometrist.

• Provide emergency advice/appointments as required, where clinic space allows, including any related to contact lens wear.

• Supply contact lenses and solutions (if used and if included in the EyeCare Plan), at regular intervals as determined by your Optometrist.

• Give one calendar month's notice of changes or termination of this agreement in writing.

• Give 20% discount on new prescription spectacles/sunglasses and prescription lenses.

• Give 15% discount on other products, including non prescription sunglasses.

• Give a minimum of 1 month's notice of any price increase.

In addition GOLD Members receive:

You Undertake to:

• Attend for an annual contact lens examination. Failure to do so could mean temporary or permanent interruption of your contact lens supplies.

• Attend for routine contact lens checks, as advised by your Optometrist.

• To attend for appointments made, and pay for missed appointments or those cancelled with less than 24 hours notice.

• To collect your contact lenses and solutions (if used and if included in the EyeCare Plan) from the Practice, unless otherwise arranged.

• Comply with all care regimes and advice given in writing, and verbally.

• Pay in advance for 3 / 6 (delete as appropriate) month's of contact lenses and solutions, if they are included in your plan.

• Pay regular amounts by Direct Debit for a minimum of 18 months, from the first payment date. Missed payments will carry a fee. If a monthly fee remains unpaid 1 month after the due date we may terminate this contract immediately by giving you written notice.

• Give 2 month's notice, in writing, if you wish to cancel this agreement after the minimum term of 18 months.
